The score was tied 43-43 after regulation.
The win gave Vermillion a record of 2-1 while South Sioux fell to 1-3.
Broc Gauer scored 23 points, pulled down seven rebounds, made six assists and added seven steals.
The Tanagers fell behind 17-13 after the first quarter, led 32-22 at halftime, and then led 36-28 going into the fourth quarter.
"We played hard and executed down the stretch," said coach Jon Reuvers.
Vermillion hit 2 of 4 three-point tries and hit 4 of 5 free throw attempts. The Tanagers sank 23 of 44 field goal attempts, while South Sioux was 14 of 40 from the field. South Sioux sank 3 of 6 three-pointers and connected on 10 of 11 free throw attempts.
"Broc Gauer nailed down a three-pointer from the top of the key to win in overtime with 3.4 seconds left," coach Reuvers said.
