Justin (Jay) Salem was born Nov. 4, 1986 in Sioux City, IA. He died Oct. 23, 2009 in Omaha, NE.
Funeral services were held in Omaha, Oct. 29 with Rev. James Tiegs officiant.
The eulogy was given by Jay's aunt, Debra Gapp. On Eagles Wings was played and sung by his uncle, Steve Gapp. A song Goodbyes, composed and recorded by Jay was played at the closing.
Survivors include his parents, Tom and Jean Salem; brothers, Ryan Dwyer and Seth Salem; sister, Kelsey Salem, all of Omaha; grandmother, Betty McCormick, Sioux City, IA; grandparents, James and Ardiss Dwyer, Vermillion; many aunts, uncles, cousins and friends.
